Waiver Services for People With Brain Injuries
If you are a New Mexican living with brain injury you MAY be able to qualify for waiver services. You must meet Federal Medicaid Eligibility Requirements including:

Medical eligibility for low-level nursing care (need assistance in 2 areas of activities of daily living)

Maximum monthly income less than $1,889 (approximately $3,578 for a married couple) and $2000 or less in assets.

Under the age of 65 years old (born after 1943)

Medical Diagnosis- having an ICD-9 Code (International Statistical Classification of Diseases and Related Health Problems) of a brain injury.
If you qualify for waiver services you will be eligible to use your budget to create a system of supports for independence at home and in the community. Services may include, but are not limited to, transportation, therapies, home health aides, housekeeping, behavioral support, equipment and many other services and/or goods that help compensate for the challenges caused by living with brain injury.

How do I apply?
If you are interested in applying for Mi Via, you will need to call Aging and Long-Term Services Department Resource Center at1-800-432-2080. Let them know that you have a brain injury and would like to be on the list to receive long term Brain Injury services. Due to the overwhelming need for brain injury long term medicaid services, the state will now be accepting names for the waiting list. As soon as a "slot" becomes available, they will mail out to you (or you can have it mailed to a friend), the "Primary Freedom of Choice". You will need to sign the "Primary Freedom of Choice" Form A and return it to Aging and Long-Term Services. This will begin the process of applying for Mi Via- Brain Injury.
