Welcome to the Brain Injury Association of New Mexico’s Individualized Medical Assistance Portfolio!
The Individualized Medical Assistance Portfolio, or IMAP, was created by the Brain Injury Association of New Mexico through a grant from the New Mexico Department of Health’s Trauma Systems Fund Authority. In creating the IMAP, it was a pleasure working with the various support groups, medical professionals and community members familiar with brain injury. This project took many months and a special thank you goes to Jen Strand, our University of New Mexico Occupational Therapy Intern, for all her research, organization and development work towards the creation of the IMAPs.
The IMAP is intended to help:
1. Organize your medical paperwork,
2. Get you prepared for the medical visit,
3. Educate your medical professionals about brain injury and how to support you,
4. Provide a means to keep you organized, so that it enhances your follow up care.
Each section is color coded to easily put back the documents in the folder. Please click onto the documents below to make copies of what you need:
____ Medical History
____ Medical Visit
(Tip Card)
____ Personal Information
____ Insurance
____ Brain Injury Information
We have tried to think of everything that you might need to help you enhance your health and healthcare. The IMAP is not perfect, and we expect that there will be some additions and improvement being made throughout the year. Please do give us some feedback and if we randomly select your name for a follow up survey we would appreciate your participation.
We hope the IMAP does help you with your medical needs. We wish you well on your journey to better medical health!
Sincerely,
Clara Holguin
